When you're scheduled for surgery, you will receive an invitation to sign up for MyChart. If you already have an active MyChart account, you will receive an email informing you to login to MyChart to complete a History Questionnaire. Three (3) days prior to your appointment, you will receive another email to eCheck-in. After surgery, you will receive a Post Op Questionnaire.
